New Patient Checklist

It is important to establish a good relationship and feel comfortable with your pediatrician. At Kintiroglou Pediatrics, we strive to create a trusting relationship with each of our families and provide the best medical care. 


Please read our checklist in "Are We A Good Fit?" to be sure we are the appropriate medical home for your family. 


For any additional questions, feel free to call our office and speak to one of our staff members.

First Appointment

A few weeks before delivery, call the office to let us know you are expecting and we can schedule a prenatal phone call with one of our doctors to answer all of your questions. You may want to download and fill out our paperwork at that time. When the happy day has arrived, you can tell the hospital staff that you have chosen us as your pediatrician.

What to Bring
  • Any paperwork from the hospital, including hearing screen results and immunization records.
  • Any available insurance information (often only the parent's insurance is available at the first visit).
  • Plenty of diapers, an extra set of clothes and a light blanket.
  • Please complete the new patient forms (you will receive a link via text) prior to the visit to help make the appointment as smooth as possible.
